Please print and complete all five pages, then bring them with you to your appointment. If you have any questions about the forms or your upcoming visit, our team is happy to help.
What to Bring to Your First Dental Appointment
Along with your completed new patient forms, please bring a photo ID, your current insurance information if applicable, a list of medications you are taking, and any dental records or X-rays you have been asked to provide.
Our Santa Fe dental team will take the time to get to know you, discuss your oral health concerns and goals, and help you feel comfortable with your care.
